Requirements

  • Current LPN License in State of operations.
  • LPN Diploma & State License Required
  • Working knowledge of Google Products
  • Current LPN License
  • BLS
  • AED

Nice To Haves

  • Knowledge of drug and alcohol treatment preferred.

Responsibilities

  • Assessment/evaluation of patient withdrawal followed by the execution of appropriate medication detox protocols; administration of prescribed medications and patient education.
  • Provide patients with a safe medically managed detox by assessing signs and symptoms of withdrawal.
  • Monitors the effects of medications administered and documents accurately and appropriately.
  • Reports and documents medication errors, pharmacy errors, packaging errors, medication count concerns, etc.
  • Ensures that ongoing assessments are conducted as determined by the patient’s condition using COWS & CIWAS and re-prioritizes care accordingly.
  • Maintains neat and accurate narcotic control inventories and all documentation related to the administration of medications.
  • Promotes patient's independence by establishing patient care goals, teaching patient to understand condition, medications, and self-care requirements, answers patient questions.
  • Assures quality of care by adhering to therapeutic standards; measuring health outcomes against patient care goals and standards; making or recommending necessary adjustments; following philosophies and standards of care set by State Board of Nursing, and other governing agency regulations.
  • Resolves patient problems and needs by utilizing multidisciplinary team strategies, including de-escalation techniques.
  • Documents patient care services by charting in EMR.
  • Maintains continuity among teams by documenting and communicating actions, irregularities, and continuing needs.
  • Contributes to team effort by accomplishing related results as needed.
  • Participates in the onboarding of new nurses for medication administration.
  • At end of shift communicate to incoming LPN pertinent data regarding patient activity and conducts medication count.
  • Treats patients, and colleagues with dignity and respect; honors each person’s right to privacy/maintains very strict patient and colleague confidentiality; respects all individual differences; supports cultural diversity.
  • Knows role and responds appropriately to all codes related to emergency situations.
  • Consistently arrives to work on time and adheres to attendance policies.
  • Other duties, as needed.
