About The Position
Advanced Care Partners is seeking a Licensed Practical Nurse (LPN) to provide one-on-one care for medically fragile children and adults in home and school settings. This role offers the opportunity to deliver high-quality, individualized care in a more focused and meaningful environment.
Requirements
- Active LPN license in Georgia or Nurse Licensure Compact
- A minimum of one year of LPN experience required.
- Current Basic Life Support (BLS) certification through the American Heart Association (AHA) or an equivalent accredited organization required.
- Valid PPD skin test
Nice To Haves
- Vent experience preferred
Responsibilities
- Deliver skilled nursing care, including G-tubes, tracheostomies, ventilators, and central lines
- Monitor patient condition and respond appropriately to changes
- Educate patients and families on care plans and procedures
- Collaborate with interdisciplinary team members to ensure optimal outcomes
