About The Position
Provide Meaningful Care Beyond the Bedside. Join Summa Home Health as a Licensed Practical Nurse and make a direct impact by helping patients recover safely and comfortably in their own homes. As a Home Health LPN, you’ll provide one-on-one skilled nursing care, build meaningful relationships with patients and families, and work alongside a supportive interdisciplinary team focused on positive patient outcomes. Alternate Solutions Health Network collaborates with health systems in joint venture partnerships to create a post-acute care solution to deliver efficient centralized operations that is patient focused. Requirements
- Current Licensed Practical Nurse (LPN) license
- Minimum of 1 year of LPN experience in an acute care setting
- Valid driver’s license and active auto insurance
- Strong communication, time management, and documentation skills
- Compassionate, dependable, and patient-focused
Nice To Haves
- Home health experience preferred
Responsibilities
- Provide skilled nursing care in patients’ homes
- Follow physician orders and care plans created by the Case Manager
- Monitor patient conditions and report changes
- Administer medications and treatments as prescribed
- Educate patients and families using teach-back methods
- Help reduce hospital readmissions through patient education and preventative care
- Complete clinical documentation in accordance with agency and Medicare guidelines
- Communicate schedule or availability changes with the appropriate team members
