About The Position

The Licensed Practical Nurse performs duties to ensure prompt, courteous, and appropriate patient care in conjunction with office and other clinical staff at The Cancer & Hematology Centers. The role supports community oncology care and may involve working at other CHC sites based on clinic and patient needs.

The LPN handles patient-care communications, supports injection clinics, administers injections, follows established plans of care, assists with scheduling and infusion support, and collaborates with pharmacy, nursing, and social work on medication assistance and management programs. Duties and responsibilities may change at any time, with or without notice.

Requirements

• Hold an LPN license in the State of Michigan.

• Preferred oncology outpatient or acute care experience.

Responsibilities

• Handle telephone messages from patients, hospitals, physicians, and pharmacies regarding patient care, plans of care, disease processes, and medication refills, in conjunction with physicians and registered nurses.
• Work through the inbox and staff the injection clinic by performing physical assessments, assessing laboratory values, administering injections, and establishing follow-up plans and future appointments.
• Refill non-oncolytic and unscheduled medications according to policy.
• Follow the patient plan of care established by a physician, nurse practitioner, or registered nurse.
• Function as a team member and expand responsibilities as directed by management to facilitate optimal workflow within the cancer center.
• Provide clinical support to the scheduling department so appropriate tests are arranged per physician request.
• Provide support to the infusion medical assistant role as needed.
• Collaborate with pharmacy, nursing, and social work to manage patients receiving medicines through patient assistance programs and patients required to enroll in medication management for oral medications.
• Work at other CHC sites when clinic and patient needs require.
• Perform other duties and activities as assigned or as needs change.
