About The Position
This is a Licensed Practical Nurse (LPN) position in skilled nursing, offering a unique style of care at Eddy Village Green. We have overnight and day LPN openings, with 12-hour shifts and an every-other-weekend commitment. This role is for full-time night LPNs, working 7 PM to 7 AM, three times a week, with an every-other-weekend commitment. St. Peter's Health Partner's is dedicated to caring for more people in more places. Newly licensed nurses are welcome to apply. Eddy Village Green is a 192-bed skilled nursing facility that provides care in a non-traditional way through 16 twelve-bed houses. The Green House model is a pioneering effort in long-term care, aiming to give elders control over their lives by honoring their lifelong routines and preferences. This model also fosters a relationship shift between nurses and the Shahbaz who manage the individual houses. Central to The Green House® Project's mission are small-scale, self-contained, and self-sufficient nursing home and assisted living settings that prioritize elders. Each home features private rooms and bathrooms, a living room with a fireplace, and easily accessible outdoor spaces.
Requirements
- A current license to practice as a Licensed Practical Nurse (LPN) in the State of New York
Responsibilities
- In collaboration with the RN collects data about residents and determines priorities is resident care based upon identified needs and goals.
- Contributes to the plan of care through collaboration and effective communication with the RN and other members of the healthcare team.
- Under the direction of the RN and within the scope of practice, complies with established nursing policies, procedures and standards to provide safe, quality care to residents.
- Administers medications and monitors IV therapy as ordered and according to established policies and standards.
- Provides emotional support to residents and their families.
- Will go between houses on campus to provide care and pass medications to residents.
