About The Position

Home Care Licensed Practical Nurse (LPN): Enjoy independence, autonomy, and flexibility in the field to visit patients one on one in their home environment. The LPN will provide care coordination, disease and medication management, skilled wound care, catheter management, ostomy education, infection prevention, and home safety. Requirements

  • Current license as an Licensed Practical Nurse (LPN) in the State of MN. License is active and in good standing.
  • Able to travel within geographic area serviced by the office.
  • Valid driver's license, reliable transportation, and proof of auto insurance are required.
  • CPR Certification

Nice To Haves

  • 2 years experience as an LPN is preferred.

Responsibilities

  • Provide care coordination, disease and medication management, skilled wound care, catheter management, ostomy education, infection prevention, and home safety.
  • Participate in ongoing interdisciplinary assessment of the patient.
  • Provides services that are ordered by the physician as indicated in the plan of care.
  • Recognizes and reports changes in the patient’s condition to the RN.
  • Participates in the development and evaluation of the plan of care in partnership with the patient, representative (if any), and caregiver(s).
  • Counsels and educates the patient and family in meeting healthcare related needs.
  • Completes timely and accurate clinical notes including addressing patient’s progress.
