About The Position

At Frankfort Terrace, a 120-bed Intermediate Care Facility (ICF) specializing in behavioral health, Nurses play a key role in helping residents feel safe, respected, and supported as they work toward their clinical and recovery goals. This role involves providing safe, compassionate nursing care in a behavioral health setting, administering medications and treatments, observing resident progress, supporting CNAs, and helping to maintain a calm, therapeutic environment. The position offers the opportunity to deliver meaningful nursing care to a consistent resident population, collaborate with a multidisciplinary team, work within clear clinical protocols, use iPads for documentation, and strengthen behavioral health nursing skills. The facility is located in a quiet neighborhood, holds a 5-Star Quality rating, and fosters a supportive team atmosphere.

Requirements

  • Licensed Practical Nurse (LPN) license in IL
  • Prior floor nurse experience within a skilled nursing facility is strongly preferred
  • Experience with the post-acute patients – specifically the cardiac, post op ortho & the medically complex
  • Electronic medical records experience is helpful (Point Click Care or PCC is ideal)
  • New Grads are encouraged to apply

  • Registered Nurse (RN) or Licensed Practical Nurse (LPN) license in IL

Responsibilities

  • Provide safe, compassionate nursing care in a behavioral health setting
  • Administer medications and treatments in accordance with individualized care plans
  • Observe resident progress and respond thoughtfully to changes in condition
  • Support and guide CNAs while working together as a unified care team
  • Help maintain a calm, structured, and therapeutic environment, using de-escalation skills when needed
  • Deliver meaningful nursing care to a consistent, structured resident population
  • Collaborate closely with providers, therapists, and care coordinators
  • Work within clear clinical protocols with strong leadership support
  • Use iPads for efficient, real-time bedside documentation
  • Strengthen and grow your behavioral health nursing skills
