This role offers a work-life balance that's hard to find, with a consistent Monday–Friday schedule and no evenings, weekends, or on-call responsibilities. The position involves predictable, local travel, shared between two nearby communities (just minutes apart), with a consistent rotating schedule: Week 1: 3 days at Location A, 2 days at Location B; Week 2: 2 days at Location A, 3 days at Location B. You will have the opportunity to build meaningful resident relationships and foster strong, lasting connections with residents and families in a supportive, community-based setting. You will work alongside experienced caregivers and leaders who value collaboration, communication, and respect. As a Licensed Practical Nurse (LPN), you will play a key role in delivering high-quality, resident-centered care across two Assisted Living and Memory Care communities. You'll collaborate closely with caregivers, leadership, and healthcare providers to ensure safe, compassionate, and personalized care.

Requirements

  • Graduate of an accredited practical nursing program
  • Active Minnesota LPN license in good standing

Responsibilities

  • Provide direct nursing care, including medication administration, treatments, and assessments
  • Monitor resident health and communicate changes to the care team
  • Assist in developing and updating individualized care plans
  • Support residents with memory loss through patience, empathy, and engagement
  • Build strong relationships with residents, families, and team members
  • Maintain accurate and timely clinical documentation
  • Mentor and support caregivers and new staff
  • Ensure compliance with state regulations and company policies
