Requirements
- Current Licensed Practical Nurse (LPN) license in the state of Minnesota.
- CPR certification required within 6 months of hire.
- Ability to work effectively with a diverse population, including residents with varying values, backgrounds, and capabilities.
- Physical capability to perform job duties.
- Proficiency in reading, writing, and speaking English to communicate effectively with residents, families, and team members.
- Ability to pass state mandated background check.
Nice To Haves
- Experience working with seniors in long-term care, home care, or assisted living settings is preferred.
Responsibilities
- Obtain necessary information to initiate and update the Plan of Care for each resident.
- Observe and document changes in resident’s physical, mental, or emotional status.
- Communicate resident changes with the interdisciplinary team and document all observations.
- Administer medications as prescribed or other physician-ordered treatments.
- Ensure accurate transcription of new physician orders and maintain clean, organized med carts and nursing stations.
- Educate residents about their medications, treatments, and side effects while respecting their right to make personal choices.
- Communicate changes in resident health to families, offering support and comfort during difficult times.
- Monitor and provide work direction to Nursing Assistants, offering guidance, instruction, and mentorship.
- Other duties as assigned.
