Requirements
- Graduate of an accredited Certificate, Diploma, or Associate School of Nursing
- Current Indiana LPN license
- Minimum of 1 year of nursing experience in a clinical care setting
- Ability and willingness to travel within assigned territory
- Valid driver's license, current auto insurance, and reliable transportation
- Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment
Nice To Haves
- Home Health or Hospice experience preferred
Responsibilities
- Provide skilled nursing care in accordance with physician orders and care plans
- Review and follow the patient Plan of Care before each visit
- Monitor patient conditions and communicate changes with the RN Case Manager and care team
- Educate patients and caregivers on treatment plans and self-care techniques
- Support patient safety initiatives and identify opportunities to prevent avoidable hospitalizations
- Administer treatments and document care accurately and timely
- Collaborate with physicians and interdisciplinary team members to ensure quality patient care
- Deliver compassionate care and exceptional service to patients and families
