About The Position

Concierge Home Care is hiring a Home Health Licensed Practical Nurse (LPN). This role offers the opportunity to take control of your schedule while providing meaningful one-on-one care in patients' homes. The coverage area is The Villages / Sumter County, FL, with territories designed to minimize drive time through clustered scheduling. The position operates on a pay-per-visit model with consistent patient volume provided.

Requirements

• Active, unencumbered Florida Licensed Practical Nurse (LPN) license
• Valid driver’s license, reliable transportation, and auto insurance
• CPR certification

• Home health experience preferred

Responsibilities

• Provide skilled nursing care under the supervision of a Registered Nurse
• Perform patient assessments and assist with care planning
• Administer medications and treatments as ordered
• Educate patients and caregivers on disease management and care plans
• Monitor patient condition and report changes to the supervising RN
• Collaborate with physicians and interdisciplinary teams
• Complete documentation using streamlined, AI-assisted tools
