About The Position
Seeking a dedicated and experienced Licensed Practical Nurse (LPN) to support patient-centered care within a clinic setting.
Requirements
- Active Minnesota LPN License required
- Strong patient care and assessment skills
- Excellent communication and interpersonal abilities
- Ability to work effectively in team-based healthcare settings
- Strong organizational skills and attention to detail
- Ability to adapt in fast-paced clinical environments
Nice To Haves
- Minimum 2 years of LPN experience preferred
- Clinic or outpatient experience preferred
- EPIC EMR experience preferred
Responsibilities
- Perform and document patient assessments within LPN scope of practice
- Assist with patient health histories and admission processes
- Support development and implementation of patient care plans
- Monitor patient conditions and report changes appropriately
- Administer medications, treatments, and nursing interventions safely
- Maintain accurate and timely patient documentation
- Assist with discharge planning and patient education
- Communicate effectively with patients, families, and healthcare teams
- Support patient education and follow-up care instructions
- Maintain compliance with facility policies and standards of care
