Licensed Practical Nurse (LPN) Night
About The Position
SSM Health Neuro Transitional Center, a joint venture between Select Medical and SSM Health, is seeking a Full-Time Licensed Practical Nurse (LPN) for night shifts. This role involves supervising Neuro Rehabilitation Coaches (NRC) and providing direct patient care within the neuro transitional rehab team. The LPN will assist NRCs in executing treatment plans, ensuring patient safety, and serving as an educator for patients, families, and coworkers. The position offers a competitive hourly rate, a significant night shift differential, and a $10,000 sign-on bonus with no repayment required.
Requirements
- Licensed Practical Nurse (LPN)
- Current State Driver’s License in good standing
- Certified BLS completion
- 3-5 years of experience
Nice To Haves
- Two years of experience in a residential care facility or rehabilitation environment
- Experience with the brain injury population
- Experience as a medication aide
- Ability to delegate and problem-solve effectively
Responsibilities
- Supervises Neuro Rehabilitation Coaches (NRC), provides guidance, and communicates staff concerns to leadership.
- Follows through on and supervises NRCs regarding patient plans of care, assisting in rehabilitation activities to promote education and patient engagement.
- Assists patients in achieving goals in various environments, including home, therapy, and community.
- Coordinates with leadership to ensure appropriate staffing levels.
- Assists patients with meal planning, food preparation, and grocery shopping; prepares inventory.
- Carries out suite management and housekeeping tasks.
- Coordinates patient transportation and transports patients as needed, ensuring adherence to transportation policies.
- Attends scheduled trainings and/or meetings.
- Completes documentation and supervises NRCs to ensure required documentation according to policy.
- Performs nurse delegation tasks including vital signs, gastric tube management, diabetic management, O2 administration, crushing medications, intermittent catheterization, catheter care, bowel and bladder program management, and trach care.
- Responsible for medication administration for a maximum of 12 patients.
