About The Position

At Tennessee Quality Care, we’re searching for a compassionate Licensed Practical Nurse (LPN) to join our Home Health team full-time. Step beyond the traditional clinical setting and deliver meaningful, one-on-one care – helping patients heal, manage chronic conditions, and maintain independence right where they feel most comfortable: at home.
Requirements
- Active LPN/LVN license in the state
- Graduate of an accredited practical/vocational nursing program
- 3+ years of nursing experience (Home Health, Med-Surg, or Community Health preferred)
- Current CPR certification
- Reliable transportation, valid driver’s license, and auto insurance
Responsibilities
- Deliver direct patient care in accordance with the state Nurse Practice Act
- Carry out care plans developed by the Registered Nurse
- Educate patients and families on self-care and treatment plans
- Document care accurately and timely
- Coordinate with RN and care team to ensure patients receive comprehensive support
- Utilize proper techniques for equipment, supplies, and infection control
- Support patients’ physical, emotional, and educational needs in the home
