Position Overview As a Licensed Practical Nurse (LPN), you will play a vital role in delivering high-quality, resident-centered care within our Assisted Living and Memory Care communities. You'll collaborate closely with caregivers, leadership, and healthcare providers to ensure each resident receives safe, compassionate, and personalized care.

Requirements

  • Graduate of an accredited practical nursing program
  • Active Minnesota LPN license in good standing

Responsibilities

  • Provide direct nursing care, including medication administration, treatments, and assessments
  • Monitor resident health and report changes promptly to the care team
  • Assist in developing and implementing individualized care plans
  • Support residents with memory loss through patience, empathy, and engagement
  • Build strong relationships with residents, families, and team members
  • Maintain accurate and timely clinical documentation
  • Mentor and support caregivers and new staff
  • Ensure compliance with state regulations and company policies
