About The Position

Symbii Home Health and Hospice is seeking a Licensed Practical Nurse to join their team. This role offers flexible hours, minimal weekends, and consistent hours, providing an opportunity for greater life balance compared to hospital or facility settings. The position focuses on direct, one-on-one patient care, allowing LPNs to build trusting relationships and make a difference in patients' health and independence. LPNs will utilize and enhance a variety of nursing skills, including wound care, IV infusions, medication administration, and patient education. While working independently, LPNs are part of an amazing team and have support from colleagues. They will be responsible for patient care decisions in collaboration with the case manager, physician, family, and other care team members, and will also develop relationships within the broader caregiving community.

Requirements

  • Must possess a valid LPN license in the State of Idaho
  • Must hold a current CPR certification
  • Must hold a valid driver’s license and current auto insurance

Responsibilities

  • Provide direct patient care as defined in State’s Nurse Practice Act.
  • Implement plan of care initiated by the registered nurse.
  • Provide accurate and timely documentation consistent with the plan of care.
  • Collect assessment data and perform observation to report to and collaborate with RN(s) and physician(s).
  • Participate in coordination of home health services, appropriately reporting the identified needs for other disciplines (HHA, OT, PT, MSW, ST, and Dietician) to the registered nurse and/or Assistant Director of Nursing or Director of Nursing.
  • Participate in personal and professional growth and development.
  • Perform other duties as assigned by the registered nurse.
