Licensed Practical Nurse (LPN)
About The Position
Catholic Health Home Care is looking for passionate, dedicated LPNs to provide comprehensive care to patients at home. This role involves case management, assessment, planning, development, and implementation of comprehensive home health care plans. Responsibilities include measuring vital signs, administering IVs, changing dressings, cleaning wounds, and providing skilled nursing care, preventative rehabilitative procedures, and prescribed treatments. The LPN will work closely with a supportive patient care team, including physicians, therapy, social services, pastoral care, and home health aides. Patient and family education for disease management and health promotion is also a key component of the role.
Requirements
- New York State Licensed Practical Nurse (LPN) license
- Minimum of one year of recent hands on experience
- Basic Life Support Certification
Responsibilities
- Case manage, assess, plan, develop and implement a comprehensive home health care plan for their patients.
- Measure vital signs, Administer IVs, change dressings, clean wounds as needed
- Provides skilled nursing care, preventative rehabilitative procedures, and prescribed treatments with a variety of patient populations within various potentially complex home situations.
- Work closely with a supportive patient care team including physicians, therapy and social services, pastoral care, home health aides and others for your assigned caseload.
- Provide patient and family education for disease management and health promotion as directed by physician’s plan of treatment.
