About The Position

Eventide's The Linden is currently seeking a Licensed Practical Nurse (LPN) – Part-Time to work our Day shifts. This position offers a 12-hour AM schedule (7:00 AM – 7:00 PM) with every other weekend and holiday rotation. Make a meaningful impact while providing compassionate care to our residents! A 7% Part-Time Shift differential is added to the base wage. The Charge Nurse provides and ensures that the highest degree of nursing care is provided by the current federal, state, and local standards, guidelines, and regulations, as well as Eventide's philosophy, policies, and procedures.

Responsibilities

Assess residents’ needs and health status through physical assessments, observations, and interviews continuously.
Provide individual attention, which encourages each resident’s ability to maintain or attain the highest physical, mental, and psychosocial well-being.
Observe and follow the residents’ plans of care. Update and make appropriate changes in plans of care whenever necessary. Plans of care include interdisciplinary care plans, physician orders, and C.N.A/NA-R care plans.
Administer medications and treatments and monitor both for effectiveness.
Provide direct nursing care to residents whenever necessary and according to established nursing policies, procedures, and standards.
Comply with rules and regulations as set forth by the Minnesota or North Dakota Department of Health and the Center for Medicare/Medicaid services.
Provide accurate documentation in the residents’ electronic medical records according to established nursing policies, procedures, and standards.
Assist in orientation/training of new team members.
Report resident needs, problems, and observations regarding residents’ condition(s) to the Charge Nurse and/or attending physician.
Ensure adequate staffing levels for a scheduled shift and correct inadequate staffing appropriately.
Monitor the activities and functions of subordinate staff, which may include participating in the disciplinary process.
Ensure that work responsibilities of subordinate staff are completed thoroughly and accurately within scheduled shifts and according to established resident care policies, procedures, and standards.
Provide information, education, and support for assigned residents and their families.
Communicate the needs of residents to resident care staff in the unit, as appropriate.
Interact with physicians and other health care facilities as needed, according to the needs of the residents.
Continually update nursing skills and knowledge by attending in-services, continuing education programs, and staff meetings.
Coordinate and participate in resident care conferences and communicate information effectively to families.
Assume responsibility for promptly reporting any incidents or evidence of abuse, neglect, or violation of residents’ rights per policy.
Be accountable for managing electronic timecards daily.
Carry out or assist with other tasks in addition to the duties listed in this job description as occasionally required.
