About The Position

Radius Community Health & Healing provides interdisciplinary, team-based, integrated, comprehensive, and person-centered primary health care for vulnerable Edmontonians with complex needs. Through primary care and health promotion, Radius Community Health & Healing improves the quality of life and well-being of populations and individuals who face multiple barriers to accessing health services. Radius achieves this by providing the highest standard of evidence-informed and client-focused inter-professional care. We collaborate with our clients to address the broad determinants of health. Licensed Practical Nurses (LPNs) play a critical role within the Radius team. Our clinical teams consist of skilled professionals, including nurses, physicians, nurse practitioners, medical office assistants, outreach workers, health advocates, mental health therapists, and other specialists, all working together to provide comprehensive, wrap-around care to Edmonton’s most marginalized populations. At Radius, LPNs are empowered to practice to their full scope within a primary health care setting, serving as leaders who demonstrate a high level of independence and collaboration. They work closely with other team members to ensure that every client receives wrap-around care, addressing their medical, social, and psychological needs with compassion and expertise. Current opportunities exist for full-time, part-time, temporary, and casual Licensed Practical Nurses (LPNs) in various programs including the Main Clinic and Outreach Team, Supervised Consumption Site, Respite to Recovery (R2R), Pathways, STBBI, and the Navigation Centre.

Requirements

  • Active registration with the CLHA (College of Alberta Licensed Practical Nurses and Health care Aids) including disclosure of any practice restrictions.
  • Ability to balance multiple priorities and work under pressure with strong problem-solving skills.
  • Knowledge of community resources in Edmonton, such as social support services.
  • Strong computer proficiency with Microsoft 365 suite, experience with MedAccess EMR an asset.
  • Excellent verbal and written communication skills.
  • Must be able to operate with a trauma-informed approach.
  • Understanding and ability to apply principles of Harm Reduction.
  • Police Information Check Including the Vulnerable Sector Check - Must be current and must be renewed every 3 years.
  • Intervention Record Check - Must be current and must be renewed every 3 years.
  • Immunization Record & Mantoux Testing
  • CPR Registration - Must be currently active.
  • Registration with CLHA - Including disclosure of any practice restrictions.

Nice To Haves

  • Experience with marginalized populations, including those experiencing homelessness, addiction, and mental health challenges, is an asset.
  • Knowledge of and experience working within a trauma-informed, harm-reduction model of care is an asset.
  • Non-Violent Crisis Intervention training, Mental Health First Aid, and Applied Suicide Intervention Training are considered assets.
  • Excellent understanding of cultural awareness is an asset.

Responsibilities

  • Work collaboratively within a multidisciplinary team to provide healthcare services in various clinical and/or community-based settings.
  • Provide direct nursing care including but not limited to assessments, triage, wound care, foot care, immunizations, specimen collection, medication administration (oral and injectable medications).
  • Overdose prevention and response.
  • Manage patient appointments and maintain accurate medical records using Electronic Medical Records (EMR).
  • Provide client education.
  • Engage with clients to foster relationships, providing advocacy and facilitating connections to social and medical supports.
  • Client Advocacy.
