About The Position

The Licensed Practical Nurse (LPN) provides person-centered nursing care to people living in an assigned group home, under the guidance of the Nurse Case Manager/Delegating Nurse (RN). The LPN works with the Regional Manager and Direct Support Professionals (DSPs) to promote independence and community inclusion for people served. Responsibilities include delivering care aligned with each person's Nursing Care Plan and Individual Plan, supporting Activities of Daily Living (ADLs), coordinating medical appointments and immunizations, and maintaining health documentation in iCM and other electronic systems. The LPN must meet DSP qualifications and complete all required training. This is an essential position, and staff may not leave their shift until appropriate coverage has arrived.

Requirements

  • Current Maryland LPN license issued by the Maryland Board of Nursing.
  • At least one year of experience providing nursing services for people with disabilities.
  • Valid Maryland driver's license with no more than two points.
  • Ability to complete all required Fello and Developmental Disabilities Administration (DDA) trainings.

Responsibilities
