About The Position

Lancaster Rehabilitation Hospital is seeking a Licensed Practical Nurse (LPN) to join their team. This is a full-time position working nights on a 3x12 schedule (6:30 PM - 7:00 AM). The LPN will provide direct patient care under the supervision of a Registered Nurse, administering treatments, assisting in assessments, and supporting interdisciplinary care planning. The role operates within a modern inpatient rehabilitation environment with a collaborative interdisciplinary care model and favorable nurse-to-patient ratios. The hospital is part of Lifepoint Health and is backed by Penn Medicine Lancaster General Health and Lifepoint Rehabilitation, focusing on restoring independence and improving lives for patients recovering from various conditions.

Requirements

  • Current Pennsylvania LPN license
  • Graduate of an accredited Licensed Practical/Vocational Nurse program
  • CPR/BCLS certification required
  • Minimum six months of Medical/Surgical experience in an acute care setting preferred.
  • IV Certification is highly preferred or must be obtained within 6 months.

Responsibilities

  • Administer medications safely, accurately, and based on patient age and clinical condition.
  • Minimize potential for medication errors by following proper medication administration procedures.
  • Assist the RN with initial assessments and patient reassessments per policy.
  • Identify and implement appropriate rehabilitation nursing interventions.
  • Perform timely and accurate Quality Improvement (QI) assessments under RN supervision.
  • Deliver care based on patient response to treatment and update the interdisciplinary plan accordingly.
  • Assess and reassess pain; use appropriate pain management techniques and educate patients and families.
  • Perform all patient care tasks in a manner that promotes patient safety and reduces medical errors.
  • Develop and implement individualized teaching plans based on patient and family learning needs.
  • Perform other duties as assigned.
