You'll provide home-based nursing care for patients as directed by the attending physician and by following the order sets and goals created by the Case Manager. Our success helping patients recover comes from engaging with them. We start at the facility bedside and coordinate each patient's transition back to their home, ensuring a smooth transition and that their care plan is right for them. We support complex, high-acuity, medically-at-risk aging patients. This is the most challenging group of individuals to serve, and we find it the most rewarding. And we do this all in partnership with health systems. Together we create a seamless care environment that enables patients to receive excellent care in the setting that best meets their needs.

Requirements

  • Licensed Practical Nurse (LPN) with current license in the state of employment.
  • Minimum one-year experience as a LPN in an acute care setting.
  • Valid driver's license and auto insurance in your name as a driver.
  • Capable of all physical demands.

Nice To Haves

  • Home care experience preferred.

Responsibilities

  • Provide home-based skilled nursing care to patients as directed by the attending physician and by following the order sets and goals created by the Case Manager.
  • Observe and monitor patient conditions and notify case manager of any changes in the patient’s condition.
  • Administer medication as prescribed by the physician.
  • Help decrease re-hospitalizations by educating the patient on preventative measures and making good decisions.
  • Educate the patient and family on the disease process using teach back methods to ensure patient and family understanding.
  • Complete all clinical documentation following agency protocol and Medicare/Federal guidelines.
  • Understand and follow agency policies, procedures, rules, and regulations.
  • Communicate changes in schedule/availability to schedulers or supervisors.
  • Attend in-service training and mandatory agency meetings.
  • Work autonomously and be efficient in managing your time.
  • Be responsible for all practices and duties within the scope of practice outlined by the state.
