About The Position

The Licensed Practical Nurse (LPN) will provide services at Branch Health Clinic Dam Neck, VA. This role is designated as mission essential and requires the contract worker to be a US Citizen. Services are to be provided Monday through Friday between 0700 and 1530, with shifts greater than six hours including an uncompensated thirty-minute meal break. The clinic is closed on all Federal Holidays, and contract HCWs will not be required to work on these recognized holidays. Substitute contract workers will only be used if the permanent contract worker is absent for more than 3 calendar days. Contract workers will be compensated for unplanned closures due to reasons beyond the contractor's fault. Telework arrangements are not permitted.

Requirements

  • Graduation from a State Board of Nursing approved LPN/LVN training program or completion of equivalent military training that permits sitting for the State licensure examination.
  • Possess a minimum of 1 year experience as an LPN working in an outpatient acute care (hospital based) or ambulatory care setting within the last two years.
  • Possess a current unrestricted license to practice as an LPN/LVN in anyone (1) of the fifty (50) States, the District of Columbia, the Commonwealth of Puerto Rico, Guam or the U.S. Virgin Islands.
  • Must be a US Citizen.

  • As an alternative to the education requirements in the basic contract, the HCW may be a graduate of an accredited LPN program by ACEN.

Responsibilities

  • Maintain a level of productivity comparable with that of other individuals performing similar services.

  • Perform timely, accurate, and concise documentation of patient care.
  • Operate and manipulate automated systems such as CHCS, AHLTA, Essentris, MHS Genesis and Clinical Information System (CIS).
  • Participate in clinical staff Performance Improvement (PI) and Risk Management (RM) functions.
  • Maintain DOD email account as directed and be responsive to all email and voicemail communications.
  • Exercise appropriate clinical delegation of tasks and duties.
  • Maintain documentation of all treatment provided in accordance with clinic directives.
  • Ensure patient care is carried out in accordance with the Standards of Nursing Care and the policies and procedures of the clinic.
  • Check patients into the clinic and triage using clinic guidelines.
  • Obtain and document patient and pertinent family history.
  • Perform a full range of diagnostic support duties which include taking vital signs, collecting specimens, obtaining, recording and tracking results of diagnostic tools.
  • Assist the physician in a variety of diagnostic examinations.
  • Perform laboratory tests such as checking urine for sugar and stool for blood.
  • Perform a range of treatment procedures that include sterile dressing changes, applying compresses, monitoring IV fluids, inserting catheters and suctions, inserting nasogastric tubes, administering medications, giving enemas, setting up and giving treatment that requires auxiliary equipment such as oxygen and suction.
  • Prepare patients for tests, examinations, treatments, and/or surgery.
  • Prepare, start, monitor and discontinue intravenous fluids with accuracy and in accordance with established procedures.
  • Observe, report and document all observed symptoms, reactions, treatments and changes in the patient condition to the registered nurse or physician.
  • Perform routine nursing care activities such as taking blood pressures, temperatures, baths and hygiene care, passing and removal of trays, changing of linen, and otherwise assist in the care of the physical needs of the patient.
  • Operate basic equipment required in delivery of patient care such as pumps, intravenous (IV) pumps, oxygen administration apparatus and incentive spirometers.
  • Execute physician’s orders within the guidelines of standard nursing practice.
  • Maintain records of nursing care, dose and time of medication administered, and indicate if the medication was not administered and the reason.
  • Administer immunizations in a safe and accurate manner with strict adherence to all applicable immunization policies.
  • Recognize emergency situations and assist with, or institute emergency measures for sudden adverse developments in patients such as cardiac arrests.
  • Perform preoperative procedures for minor surgery and fill out pre-op checklist.
  • Assist patients in admission, transfer, and perform discharge planning follow-up and documentation.
  • Support the patient and/or family members toward the achievement of treatment plan goals.
  • Provide instructions to the patient on invasive procedures, surgical procedures and post-surgical conditions.
  • Instruct patients on how equipment is used such as oxygen, suction, cardiac monitor and pulse oximeter.
  • Provide emotional support to patients and families.
  • Ensure necessary supplies are available and equipment is in functioning order.
  • Provide an orderly, clean and safe environment for patients and staff.
  • Handle telephone information requests with courtesy, accuracy and respect for patient confidentiality.
  • Receive information and distribute messages as necessary.
  • Report to supervisor any need for maintenance or re-supply of pharmaceuticals.
  • Ensure upkeep and perform checks of emergency equipment i.e., oxygen, emergency cart, suction apparatus, etc. and maintain appropriate logs.
  • Perform additional duties within the scope of LPN practice including but not limited to, update of Standards of Nursing Care, and other collateral duties required for efficient, quality patient care.
  • Transport patients to and from other areas of the facility to include hospital clinics and ancillary departments, as ordered by the physician.
  • Assist the professional nurse and/or physician with individual, group, and family health instruction.
  • Initiate and maintain pertinent patient and family medical history on patients, recording observations and stated symptoms for use by the Medical Home Port (MHP) Team.
  • Utilize automated programs and information technology tools to facilitate the patient experience and screen Medical Home Port patients, needing preventative services.
  • Ensure data entry for population health metrics and health promotion, while assisting in the maintenance and update of patient registries.
  • Utilize the patient’s preferred method of communication to provide integrative and comprehensive care and use clinic available tools for proactive patient management of their health care needs.
  • Track HEDIS measures, lab/rad results and notify patients per command/department policies.
  • Check patient identification, physician's orders, and transfer information as applicable.
  • Requisition and arrange for ordered diagnostic and therapeutic services as directed.
  • Ensure review of emergency treatment records (ETR) and follows up with patient, per command and department policies.
  • Administer immunizations, allergy treatments, oral, parenteral, IV medications, and maintains annual competencies as outlined in departmental policies.
  • Actively participate in MHP Team huddles.
  • Proactively assists team’s identification of diagnostic procedures required during schedule scrub.
  • Assist with identification of patient education needs and reviews plan with team members for coordination of educational needs.
  • Prepare equipment and assist physicians with treatment procedures.
  • Assist with clinic check-out and follow-up visits by reviewing post-visit needs with the patient.
  • Ensure scheduling of follow-up appointments or specialty care referrals.
  • Assist patients by directing or escorting to other clinical services (laboratory, radiology, pharmacy, immunizations, etc.), as required.
  • Participate in the peer review and training of staff and maintenance of the Medical Home Port Team Accreditations (NCQA, JC, IG etc.).

Benefits

  • Services shall be provided Monday through Friday between 0700 and 1530 for eighty (80) hours per week.
  • Shifts greater than six-hour (6) hours include an uncompensated thirty-minute (30) meal break.
  • Contract HCWs will not be required to work on federally recognized holidays.
  • During unplanned (unknown) closures due to no fault of the Contractor (e.g., natural disaster, military emergency, severe weather, pandemic, or Government decision to close the facility outside of a planned closure), the contract worker will be compensated for the unplanned closure (personal services only).
