Requirements

Current Licensed Practical Nurse (LPN) license in Ohio or West Virginia, as applicable.
Graduate of an accredited practical nursing program.
Current CPR certification (or ability to obtain upon hire).
PRN employees are required to maintain active employment status by working a minimum of one (1) scheduled shift per month.

Responsibilities

Provide direct nursing care to residents in accordance with physician orders and individualized care plans.
Administer medications and treatments accurately and safely.
Monitor residents' conditions and promptly report changes to the RN, physician, or appropriate supervisor.
Accurately document resident care and maintain compliance with state and federal regulations.
Collaborate with nurses, nursing assistants, therapists, physicians, and other interdisciplinary team members to provide exceptional resident care.
