LPN (Licensed Practical Nurse) – Howland Urgent Care - This is a 12 shift schedule Monday through Friday - 12 hour shifts - 8am to 8pm and a weekend shift every 3 to 4 weeks. Weekend hours are 8 to 4pm. 2 shifts one week and 3 shifts the next. Summary of Primary Function/General Purpose of Position Under the direction of a Licensed Professional (e.g., Physician or Nurse Practitioner), the Licensed Practical Nurse (LPN) is responsible for the delivery of patient care utilizing the nursing process through data collection and assessment, identifying and determining the priority of patient’s problems/needs, and evaluating the process and outcome of nursing care. The LPN coordinates, provides, and directs patient teaching activities and coordinates the care provided by health team members. The LPN complies to all documentation standards within the scope of the Medical Assistant and the LPN role.

Requirements

  • Current Licensed Practical Nurse (LPN) from the state of employment or LPN applicant (required)
  • BLS certification is preferred upon hire and required within 30 days and/or prior to independent patient care
  • 1 year of clinical patient care experience (preferred)
  • EPIC Electronic Health Record (EHR) training (preferred)

Responsibilities

  • Analyzes the assessment data, evaluates patient's condition, and collaborates with the provider to develop and maintain the patient's plan of care.
  • Provides education to patients, families, and staff members, in conjunction with other healthcare entities, under the direction of the provider or manager.
  • Coordinates the care of the patient with other healthcare entities according to the patient's plan of care.
  • Administers medications in a safe manner, which is consistent with the organizations policies and procedures as well as the state requirements with which they are licensed in.
  • LPNs in a lead position serve as subject matter experts and clinical and professional mentors within the practice.
  • Take and document vital signs
  • Room patients
  • Monitor patient conditions during in-office treatment(s)
