About The Position

The LPN uses experience, knowledge, judgment and skills to provide nursing care to patients and families in all settings. Maintains patient confidentiality and promotes patient rights as supported by agency, state and federal directives. Identifies cultural diversity and lifestyle choices and non-judgmentally accommodates for such differences. The LPN is committed to the philosophy, goals and objectives of the hospice concept. The LPN works under the direct supervision of the RN while consistently demonstrating the Hospice Savannah values and compliance with organization’s service excellence standards.

Requirements

  • Graduate of an accredited school of Nursing, with a current GA LPN License.
  • Minimum of two years of experience in hospice, oncology, med-surg or home health providing direct patient care required.
  • Current CPR certification, valid driver’s license, automobile insurance and reliable transportation required.
  • Excellent communication skills (written and verbal).
  • Ability to successfully interact with a diverse patient population, families, physicians, Hospice Savannah staff, and the community at-large.
  • Excellent customer service and documentation skills required.
  • Computer proficiency in Microsoft Office products including Outlook, and database knowledge required.

Responsibilities

  • Accurately, timely and concisely document the patient/family encounter using a computer.
