About The Position

Join our dedicated home health team as a Licensed Practical Nurse (LPN) and make a meaningful impact in the lives of individuals and families throughout your community. About the Role: As an LPN, you'll provide hands-on nursing care directly in patients' homes, supporting individuals through acute, chronic, or rehabilitative health challenges. Under the supervision of a Registered Nurse (RN), you'll play a key role in delivering high-quality care, advocating for your patients, and ensuring their care plans are accurately followed and documented. This position will cover our Northern territory, and can be based in Danbury or New Milford, CT.

Requirements

  • Graduate of an accredited nursing program.
  • Active LPN license in the State of Connecticut.
  • Minimum of 2 years clinical nursing experience (home care experience strongly preferred).
  • Strong understanding of medical terminology and Medicare/insurance documentation requirements.
  • Calm under pressure with strong organizational and multitasking skills.
  • Excellent interpersonal communication and customer service skills.
  • Proficient with electronic health records and computer documentation.
  • Must have reliable transportation and the ability to travel in all weather conditions.

Responsibilities

  • Deliver skilled, direct patient care as assigned, while communicating timely updates to the RN Case Manager, physician, and Clinical Supervisor.
  • Monitor and update nursing care plans based on physician directives.
  • Educate clients and families about diagnoses, medications, and treatment plans.
  • Use and maintain medical equipment and nursing supplies properly and efficiently.
  • Document patient information accurately using electronic medical records.
  • Participate in the on-call rotation and provide support to other clinical staff as needed.
  • Identify and escalate any emerging clinical issues to the appropriate leadership.
  • Build strong, trust-based relationships with patients, families, and team members.
  • Support agency quality improvement efforts and participate in ongoing professional development.
