About The Position

The Licensed Practical Nurse (LPN) provides direct nursing care to patients receiving inpatient psychiatric and behavioral health services. Under the direction of a Registered Nurse, physician, or other licensed provider, the LPN assists in the assessment, treatment, monitoring, and rehabilitation of patients experiencing mental health and co-occurring substance use disorders.

Requirements

  • Current unrestricted Licensed Practical Nurse (LPN) license in the State of Idaho, and must maintain licensure in good standing
  • 1 year of nursing experience
  • AVADE training and CPR certification within 90 days of hire
  • Successful completion of background check

Nice To Haves

  • Experience in psychiatric, behavioral health, substance use treatment, inpatient, emergency, correctional, or residential treatment settings
  • Basic understanding of psychiatric diagnoses, psychotropic medications, substance use disorders, trauma-informed care, and recovery-oriented treatment
  • Experience using electronic health record systems

Responsibilities

  • Provide direct nursing care to patients within the scope of Idaho LPN licensure and hospital policy.
  • Perform nursing assessments, collect patient health information, obtain vital signs, monitor weight, and document patient observations and responses to treatment.
  • Monitor patients for changes in physical, emotional, behavioral, and psychiatric status and promptly report concerns to the appropriate licensed provider.
  • Assist with patient admissions, transfers, and discharges, including completion of required nursing documentation.
  • Conduct health screenings and assist in identifying patient needs requiring further evaluation or medical intervention.
  • Perform AIMS assessments and other clinical screenings as trained and assigned.
  • Administer medications, including oral, injectable, and other prescribed treatments, in accordance with provider orders, hospital policies, and applicable regulations.
  • Monitor patients for medication effectiveness, adverse reactions, side effects, and medication compliance.
  • Maintain accurate and timely documentation in the Medication Administration Record (MAR) and electronic health record.
  • Provide patient education regarding medications, diagnoses, treatment plans, wellness, and recovery.
  • Communicate medication-related concerns, abnormal findings, and patient responses to the prescribing provider and nursing leadership.
  • Coordinate with pharmacies, medical providers, and other healthcare professionals to ensure continuity of care and accurate medication management.
  • Participate in interdisciplinary treatment team meetings and contribute nursing observations and recommendations regarding patient care and treatment planning.
  • Assist in the implementation and monitoring of individualized treatment plans.
  • Observe and monitor patients for suicide risk, self-harm behaviors, aggression, elopement risk, withdrawal symptoms, and other safety concerns.
  • Respond appropriately to psychiatric and medical emergencies.
  • Utilize approved de-escalation techniques and participate in crisis intervention procedures.
  • Complete required clinical, nursing, and regulatory documentation accurately and within established timeframes.
  • Ability to multi-task, manage time, meet deadlines, and remain calm and professional under pressure.
  • Maintain standards of confidentiality of Lifeways’ clients and records.
  • Perform all work duties and activities in compliance with Lifeways, Oregon Administrative Rules, OSHA and HIPAA regulations.
