About The Position

Lifetime Assistance is seeking a Licensed Practical Nurse (LPN) to join their Residential Services team. This full-time position involves providing compassionate, person-centered healthcare to individuals with intellectual and developmental disabilities in residential settings. The role includes administering medications, monitoring health status, assisting with personal care, coordinating medical appointments, and contributing to nursing care plans. The LPN will also respond to emergencies, act as a shift leader when assigned, and maintain accurate documentation. Requirements

  • Current New York State Licensed Practical Nurse (LPN) license in good standing.
  • CPR and First Aid certification (or ability to obtain).
  • SCIP-R Level II certification or willingness to complete training.
  • Valid NYS driver’s license and ability to meet Lifetime Assistance’s Vehicle Operator Requirements.
  • Strong communication, problem-solving, and decision-making skills.
  • Ability to follow medical directions and adapt to changing care needs.
  • Ability to lift up to 35 pounds.
  • Ability to respond to emergencies.
  • Ability to perform frequent physical activity.

Responsibilities

  • Deliver compassionate, person-centered health care to individuals in residential settings.
  • Administer medications and treatments as prescribed; monitor health status and report changes to the RN.
  • Provide personal care, support daily living routines, and promote independence.
  • Coordinate and accompany individuals to medical appointments; ensure follow-up care is carried out.
  • Assist the RN with developing nursing care plans, health-related goals, and staff training.
  • Respond to emergencies and behavioral incidents, utilizing SCIP-R strategies.
  • Act as shift leader when assigned and collaborate with interdisciplinary teams.
  • Maintain accurate documentation in compliance with agency and regulatory requirements.
  • Support a clean, safe, and welcoming home environment.
