About The Position

Vitality Hospice & Palliative Care is seeking a dedicated Per Diem Licensed Practical Nurse (LPN) to provide high-quality, patient-centered care to individuals receiving hospice services in their homes or care facilities. This per diem role offers flexibility, autonomy, and meaningful one-on-one patient care, making it ideal for nurses who want to make an impact while maintaining schedule freedom.

Requirements

• Active and unencumbered LPN license in the state of practice
• Minimum 1 year of nursing experience preferred
• Knowledge of hospice philosophy and care for terminally ill patients
• Strong assessment, communication, and documentation skills
• Ability to work independently and manage time effectively
• Valid driver’s license, reliable transportation, and current auto insurance

Responsibilities

• Provide skilled hospice nursing care in accordance with the patient’s individualized plan of care
• Perform nursing assessments and promptly report changes in patient condition
• Support pain and symptom management to promote comfort and quality of life
• Assist patients with activities of daily living (ADLs) as needed
• Educate patients, families, and caregivers on care needs and self-care strategies
• Accurately document visits and care provided in a timely manner
• Collaborate with the interdisciplinary hospice team to ensure coordinated, holistic care
• Participate in on-call rotation as available (optional, based on per diem needs)
