Human Touch Home Health is looking for compassionate and dependable Licensed Practical Nurses (LPNs) for PRN home health opportunities in Blacksburg and surrounding communities. Requirements
- Active Virginia Licensed Practical Nurse (LPN) License Required
- Graduate of an accredited Practical Nursing program
- Valid Driver's License and reliable transportation
- Strong clinical and communication skills
- Dependable, organized, and comfortable working independently in patients' homes
- Ability to follow established plans of care and communicate effectively with the supervising RN
Nice To Haves
- Home Health Experience Preferred, but Not Required
- WellSky Experience is a Plus
Responsibilities
- Provide skilled nursing care under the direction of an RN and physician-established plan of care
- Complete skilled nursing visits in patients' homes
- Monitor and document patients' conditions
- Provide medication management and education
- Perform wound care and other ordered treatments
- Monitor vital signs and report significant changes
- Reinforce disease-management education
- Educate patients, families, and caregivers
- Communicate changes in condition to the supervising RN and care team
- Coordinate with other members of the interdisciplinary team
- Follow established plans of care and physician orders
- Complete accurate and timely documentation using WellSky
