About The Position
Diamond Willow is an Assisted Living and Memory Care facility that started in 2004. Our Park Rapids, MN facility consists of a home like atmosphere with 30 residents. We are committed to providing the utmost quality of care with the emphasis on person-centered care planning.
Requirements
- Possess a valid Minnesota LPN License
- Minimum of 1 year experience
- Leadership ability
- Must be able to occasionally fill in when needed.
- Maintaining confidentiality and professionalism at all times
- Must be able to work as a team member and maintain a positive demeanor
- Positive work attitude
Responsibilities
- Medication management & chart auditing
- Performing resident treatments as ordered per the care plan
- Observing, tending to and documenting residents health needs
- Communicating with RN Director regarding relevant staff and resident information and/or concerns
- Reviewing appointments & setting up transportation for residents
- Processing MD orders completely and accurately
- Completing incident reports as needed
- Ensure the staff is performing safe practices
- Executing tours for potential admits
- Reviewing messages and communicating with staff through R-Task
