About The Position

Symbii Home Health and Hospice is seeking a Licensed Practical Nurse (LPN) to join their team. This role offers the opportunity to provide life-changing service to patients in their homes, focusing on improving health and independence. The position allows for flexible scheduling, minimal weekend work, and a better work-life balance compared to traditional hospital or facility settings. LPNs will engage in direct, one-on-one patient care, utilize a variety of nursing skills, and work both independently and collaboratively with a supportive team. The agency has been in business locally for over 12 years with 11 branches across Idaho and is part of The Pennant Group, Inc., a holding company for healthcare service providers.

Requirements

  • Must possess a valid LPN license in the State of Idaho
  • Must hold a current CPR certification
  • Must hold a valid driver’s license and current auto insurance

Responsibilities

  • Provide direct patient care as defined in State’s Nurse Practice Act.
  • Implement plan of care initiated by the registered nurse.
  • Provide accurate and timely documentation consistent with the plan of care.
  • Collect assessment data and perform observation to report to and collaborate with RN(s) and physician(s).
  • Participate in coordination of home health services, appropriately reporting the identified needs for other disciplines (HHA, OT, PT, MSW, ST, and Dietician) to the registered nurse and/or Assistant Director of Nursing or Director of Nursing.
  • Participate in personal and professional growth and development.
  • Perform other duties as assigned by the registered nurse.
