About The Position

Anchor Post Acute is seeking compassionate Licensed Practical Nurses (LPNs) to join their award-winning team. This role involves providing direct nursing care to residents and supervising Certified Nursing Assistants (CNAs) to ensure the highest quality of care is maintained in compliance with all federal, state, and facility guidelines. The facility is a 120-bed skilled nursing and rehabilitation center located in Aiken, South Carolina, recognized as Aiken's Choice Award Winner for Top Skilled Nursing Facility.

Requirements

Nursing Degree from an Accredited College or University
ACTIVE, Unencumbered South Carolina LPN License
Ability to Work 12-Hour Shifts
Ability to Work Weekends
Must be able to speak and write the English language in an understandable manner.
Must be able to see and hear or use prosthetics that will enable these senses to function adequately.
Must meet the general health requirements set forth by the policies of this facility, which include a medical and physical examination.
Must be able to push, pull, move, and/or lift a minimum of 25 pounds to a minimum height of 5 feet and be able to push, pull, move, and/or carry such weight a minimum distance of 50 feet.
Current CPR certification.
Must maintain all required continuing education/licensing.
Must remain in good standing with the State Board of Nursing at all times.

Experience in a Fast-Paced, High-Admissions Healthcare Setting Preferred
Knowledge and experience with PCC preferred.

Responsibilities

Provide direct nursing care to residents.
Supervise the day-to-day nursing activities performed by nursing assistants.
Ensure compliance with federal, state, and facility guidelines.
Administer medications and treatments as ordered by physicians.
Complete and file required recordkeeping forms and charts.
Transcribe physician's orders.
Chart nurses' notes in an informative and descriptive manner.
Participate in the development, maintenance, and implementation of the facility’s quality assurance program.
Participate in facility surveys and inspections.
Assist in planning nursing services for resident discharge.
Perform administrative duties such as completing medical forms and reports.
Admit, transfer, and discharge residents.
Complete accident/incident reports.
Write resident charge slips.
Maintain the Daily Census Report.
Prepare and administer medications.
Verify resident identity before administering medication/treatment.
Ensure narcotic records are accurate.
Participate in employee performance evaluations.
Inform supervisor of staffing needs.
Provide leadership to nursing personnel.
Make daily rounds to ensure work assignments are performed.
Ensure nursing assistants are enrolled in or have graduated from an approved training program.
Meet with nursing personnel to identify and correct problem areas.
Create and maintain a positive and calm environment.
Receive/give nursing report upon reporting in and ending shift.
Report occupational exposures.
Ensure rooms are ready for new admissions.
Greet newly admitted residents.
Participate in the orientation of new residents/family members.
Make rounds with physicians.
Requisition and arrange for diagnostic and therapeutic services.
Consult with resident’s physician.
Review resident’s chart for specific treatments and orders.
Implement and maintain established nursing objectives and standards.
Make periodic checks to ensure prescribed treatments are administered.
Ensure direct nursing care is provided by qualified personnel.
Cooperate with social and activity programs.
Notify physician and next-of-kin of changes in resident’s condition.
Carry out restorative and rehabilitative programs.
Inspect nursing service treatment areas daily.
Administer professional services such as catheterization, tube feedings, wound care, etc.
Use restraints when necessary.
Obtain specimens for lab tests.
Take and record vital signs.
Monitor seriously ill residents.
Check foods brought in by visitors.
Ensure personnel providing direct care follow the care plan.
Ensure residents who are unable to call for help are checked frequently.
Assist in arranging transportation for discharged residents.
Inform family members of the death of a resident.
Call funeral homes when requested by family.
Participate in developing and conducting in-service training classes.
Implement and maintain an effective orientation program for new employees.
Assist in standardizing work methods.
Assist in training personnel on tasks involving potential exposure to blood/body fluids.
Attend and participate in outside training programs.
Attend and participate in annual facility in-service training programs.
Attend and participate in continuing education programs.
