About The Position
Catholic Health Home Care is looking for passionate, dedicated LPNs to provide comprehensive care to patients at home. This role involves working weekend shifts from 7:00 AM to 5:30 PM (10-hour shifts) and is eligible for full-time benefits. The position covers the following areas: Bohemia, Brentwood, Fire Island, Great River, Islandia, Islip, Lake Ronkonkoma, Oakdale, & West Sayville.
Requirements
- New York State Licensed Practical Nurse (LPN) license
- Minimum of one year of recent hands on experience
- Basic Life Support Certification
Responsibilities
- Case manage, assess, plan, develop and implement a comprehensive home health care plan for their patients.
- Measure vital signs, Administer IVs, change dressings, clean wounds as needed.
- Provides skilled nursing care, preventative rehabilitative procedures, and prescribed treatments with a variety of patient populations within various potentially complex home situations.
- Work closely with a supportive patient care team including physicians, therapy and social services, pastoral care, home health aides and others for your assigned caseload.
- Provide patient and family education for disease management and health promotion as directed by physician’s plan of treatment.
