About The Position

Alvita Care is seeking experienced, Licensed Practical Nurses (LPNs) for private duty home healthcare cases throughout Central and Northern Connecticut. We offer flexible shift options, including 6, 8, and 12-hour shifts, with both day and night availability. This is a per diem position, allowing for part-time work or integration around your current schedule. You will provide 1-on-1 care in the home environment.

Requirements

CT LPN licensure
BLS, ACLS, or CPR certification
Minimum of One (1) year demonstrated experience as an LPN
Ability to put patients at ease and explain physician's instructions
Ability to multi-task with excellent organization skills
Experience working directly with people from diverse racial, ethnic and socioeconomic backgrounds
Knowledge of medical terminology
Team player and ability to work independently
Valid driver's license or NYS Non-Driver photo ID card
Successful completion of the onboarding process

Responsibilities

Consult with Management on complex/unique cases
Monitor/Assess patients/clients' condition and the effects of the care plan
Oversee/supervise the care provided to the patient by Licensed Practical Nurses and Paraprofessionals
Manage, plan, coordinate, and execute the patient's safe discharge from service
Ensure that physician's orders, discharge summary, and communication to team members, patient, and family members adhere to company and regulatory requirements
Communicate and consult with patient's MD when needed, manage and coordinate all aspects of assigned caseload
Assist patients with activities of daily living
Document content and process of all services provided in the internal system properly
Transport/utilize medical equipment to and from clients/patients' home/facilities
Travel to patient/clients' homes and/or other facilities
Perform other related duties and participate in special projects, as assigned
