About The Position

Completes the home medication reconciliation process upon admission. The LPN will work collaboratively with patients, families, physician, pharmacists, clinics, home health and other agencies to ensure an accurate and complete record of medication is maintained. This position will also be scheduled nursing shifts for the Medical/Surgical department and will practice under the supervision of a registered nurse, advanced nurse practitioner, licensed physician, or other health care provider. The LPN organizes and directs those aspects of care for which accountable; communicating and coordinating care with well-defined nursing diagnoses, including all health team members.

Responsibilities

  • Demonstrates ability to perform all essential job functions of an LPN, collects data and conducts a focused nursing assessment of the health status of a patient.
  • Obtains and assures complete and accurate medication history upon admission.
  • Assures medications are ordered upon admission; assists with medication lists on discharge as time permits.
  • Assists in orientation of Home Medication Reconciliation (HMR) process with new employees.
  • Assists with Adverse Drug Reaction Reporting Process.
  • Serves as a liaison between Pharmacy, nursing and information services to streamline HMR.
  • Completes HMR process audits and develops action plan for improvement.
  • Participates with other Avera systems to integrate and standardize processes across the continuum.
  • Communicates and collaborates with patients and members of the health care team.
  • Assists with health counseling and teaching.
  • Assists in the evaluation of responses to interventions.
  • Delegates and assigns nursing interventions to implement the plan of care.
  • Provides for the maintenance of safe and effective nursing care rendered directly or indirectly.
  • Assists in the evaluation of responses to interventions
  • Assists with CMS Indicator Compliance and other regulatory requirements.
