Licensed Practical Nurse - LPN
About The Position
This is a full-time night position (7:00 PM - 7:00 AM) at Pleasant View Skilled Nursing Community in Ottawa, IL. As the most senior clinical presence on the floor during nights, you will be responsible for providing steady, attentive care to residents, especially when conditions change rapidly. The role involves direct nursing care, monitoring resident health, responding to changes in condition, completing assessments, contributing to care plans, administering medications and treatments, communicating with providers and families, and ensuring continuity of care between shifts.
Requirements
- Active Illinois LPN license
Responsibilities
- Provide direct nursing care to residents through the night, including monitoring health status and responding to changes in condition
- Complete resident assessments and contribute to individualized care plans
- Administer medications and treatments within your scope of practice as an LPN
- Communicate condition changes to the on-call provider and to residents' families
- Keep consistent continuity of care across your assigned residents shift to shift
