About The Position

This is a full-time, 3-11 position at an Assisted Living and Memory Care Community, requiring every other weekend availability. Immediate hires are available for qualified candidates. The role is responsible for carrying out the instructions of the Director of Wellness, supervising and monitoring nursing staff to ensure the highest quality of care through routine nursing duties. The LPN is accountable to the Director of Wellness/Executive Director for all nursing department activities and maintaining the corporate philosophy of quality care. Duties include supervising HHAs, CNAs, and facility staff, making daily rounds, observing and reporting patient symptoms, administering medications and treatments, ordering supplies and medications, supervising Home Health Aides, administering emergency first aid, ensuring daily assignments are completed, documenting patient care, maintaining the cleanliness of the nurse's station and med room, assisting with new staff orientation, completing admission and discharge evaluations, checking equipment, providing guidance to nursing assistants, and promoting good interpersonal relationships. The role involves performing duties for both assisted living and independent residents, and may require performing other duties as requested by the Director of Wellness. Refusal to perform tasks may result in disciplinary action.

Requirements

Graduate of an accredited professional nursing program
Currently licensed to practice per state
Work experience in the geriatric field
Ability to communicate well and in writing.
Ability to deal with visitors, staff and residents in a courteous manner.
Ability to lift, push, pull up to 60lbs.
Strong organizational skills.
Must be able to work under pressure at times.
Must be able to perform good grooming practices, personal hygiene and be punctual and regular in attendance.
Must have warm, friendly, congenial personality.
Must present a professional appearance and possess an empathetic attitude towards seniors.
Ability to assess a resident’s condition and intervene when needed.
CPR certified, or obtained within 90 days of hire and yearly thereafter.

Responsibilities

Supervise HHAs, CNAs and the facility staff.
Make daily rounds of all Assisted Living residents and retirement residents if needed.
Observe and report symptoms and conditions of patients to Director of Wellness “DOW” /Delegating R.N./Physician.
Administer medications and treatments per signed doctor’s orders in an accurate, safe manner, observes effectiveness and reports to physician if needed.
Inform DOW of when supplies and equipment are needed.
Order medication from pharmacy when needed.
Supervise the Home Health Aide to ensure that all nursing care is given properly, safely and of the up most quality.
Administer emergency first aid.
Assure that daily assignments for patient care, incidental duties, performed by nurse and HHA as outlined by Director of Wellness are carried out.
Document patient care, including V/S, weights, nurse’s notes, medication, kardex, etc.
Make daily rounds of all patients and area.
Recognize, interpret and report conditions to the DOW/RN/ED as needed.
Maintain cleanliness and neatness of nurse’s station, med room and cart.
Assist in the orientation of new staff.
Complete admission and discharge evaluations as necessary.
Check equipment for cleanliness and condition.
Provide guidance for nursing assistants.
Promote and maintain good interpersonal relationships with residents, visitors and personnel.
Perform related work as required per assignment.
Carry out duties and responsibilities for both assisted living and independent residents throughout the facility.
