About The Position

The Licensed Practical Nurse (LPN) is responsible for medication administration for residents in compliance with Federal & State regulations. This role may involve contributing to the admission of new residents, including assessments and documentation. The LPN may also contribute to the implementation of acute and long-term care plans or service plans and/or the collection of data. Additionally, the LPN may be responsible for clinical system tracking (e.g., wounds or infection control) under the direction of the director. This position provides general consultation regarding health-related concerns within the community, notifies physicians and/or family members of any change in resident’s health with proper documentation, and responds quickly to all emergencies. Maintaining close communication with all departments and staff members, providing educational meetings for residents and staff, and understanding their role in the Safety & Disaster Plan are also key responsibilities. The LPN must maintain the confidentiality of residents’ and prospective residents’ information, attend all required training, in-service, and staff meetings, and strive to maintain a safe working environment. A positive and professional demeanor toward residents, visitors, families, and co-workers is expected. Other duties as assigned.

Requirements

Ability to commute/relocate to Clinton, CT 06413: Reliably commute or planning to relocate before starting work (Required)
LPN (Preferred)
BLS Certification (Preferred)
LVN (Preferred)

Responsibilities

Responsible for medication administration for the residents in compliance with Federal & State regulations
May need to contribute for admissions of new residents to assist all admission assessments and documentation.
May contribute to process of implementing acute and long-term care plans or service plans and/or the collection of data
May be responsible for clinical system tracking (i.e., wounds or infection control) under the direction of the director.
Provide general consultation regarding health-related concerns within the community.
Notify physicians and/or family members of any change in resident’s health and provides proper documentation.
Respond quickly to all emergencies
Maintain close communication with all departments and staff members.
Provide educational meetings for residents and staff.
Understand roll in the Safety & Disaster Plan.
Maintain confidentiality of residents’ and prospective residents’ information.
Attend all required training, in-service, and staff meetings.
Strive to maintain a safe working environment through the prevention of accidents, the preservation of equipment, and the achievement of safe working practices.
Maintain a positive and professional demeanor toward residents, visitors, families, and co-workers.
