About The Position
Human Touch Home Health is seeking a compassionate and dependable Licensed Practical Nurse (LPN) to join our growing team serving Fredericksburg, Stafford, Spotsylvania, King George, Massaponax, Locust Grove, and surrounding communities. This PRN opportunity offers flexibility, independence, and the ability to provide meaningful one-on-one patient care. As a Home Health LPN, you'll provide care in patients' homes, build relationships, educate patients and caregivers, support treatment plans, and help individuals maintain their health and independence. The role focuses on quality care, meaningful interactions, and positive outcomes, offering a contrast to traditional hospital settings.
Requirements
- Active Virginia Licensed Practical Nurse (LPN) License Required
- Valid Driver's License and Reliable Transportation
- Strong Clinical, Communication, and Documentation Skills
- Commitment to Delivering Exceptional Patient Care

- Home Health Experience Preferred but Not Required
Responsibilities
- Provide skilled nursing services under the supervision of a Registered Nurse
- Administer medications and treatments as ordered
- Monitor patient conditions and report changes to the care team
- Educate patients and caregivers on disease management and safety
- Assist with implementing individualized plans of care
- Complete timely and accurate clinical documentation
- Collaborate with physicians and interdisciplinary team members
