We are currently seeking a compassionate and dependable Licensed Practical Nurse (LPN) to join our skilled home health team serving Portsmouth and the surrounding Seacoast communities. In this role, you’ll provide direct patient care in a home setting where one-on-one attention allows you to truly connect with your patients and support their health and independence. Requirements

  • Current Licensed Practical Nurse (LPN) license in the State of New Hampshire
  • Minimum of two (2) years of skilled nursing or acute care experience preferred
  • Current CPR Certification required
  • Reliable transportation and valid driver’s license
  • Strong communication and clinical assessment skills
  • Compassionate, dependable, and patient-focused mindset

Nice To Haves

  • Home healthcare experience preferred, but not required

Responsibilities

  • Provide skilled nursing care and treatments to patients in their homes
  • Monitor patient conditions and report changes to the supervising RN or physician
  • Administer medications and treatments as prescribed
  • Maintain accurate and timely clinical documentation and patient records
  • Assist with patient assessments and individualized care planning
  • Educate patients and family members on medications, treatments, disease management, and safety practices
  • Collaborate with physicians, nurses, therapists, and caregivers to support patient goals
  • Ensure compliance with state and federal home health regulations and company standards
