About The Position

Walnut Creek Nursing and Rehab is seeking a Licensed Practical Nurse to provide care that supports residents’ health, comfort, and dignity. The role is listed as full-time and follows a weekend Baylor schedule of 6 a.m. to 6 p.m.; employees working two 12-hour shifts receive six hours of bonus pay. A $5,000 sign-on bonus is advertised. The position is based at 4850 Zuck Road, Erie, Pennsylvania. The LPN monitors residents’ vital signs, assists with daily needs, supports medical staff by administering medications and performing basic medical tasks, maintains accurate and confidential patient records, observes residents and reports health changes, communicates procedures and care plans to residents and families, and changes dressings and bandages. Requirements

Hold an active Licensed Practical Nurse (LPN) license in Pennsylvania.
Have in-depth knowledge of skilled nursing standards of care.
Have strong critical-thinking and communication skills.
Hold a BLS certification.

Responsibilities

Monitor residents’ vital signs.
Assist residents with daily needs, including bathing, dressing, and feeding, while maintaining their comfort and dignity.
Administer medications and perform basic medical tasks in support of medical staff.
Maintain accurate and confidential patient records.
Observe residents and report changes in their health to the medical team.
Communicate procedures and care plans clearly to residents and their families.
Change dressings and bandages to support proper healing.
