Requirements
- Current Licensed Practical Nurse (LPN) License in the State of New Jersey
- CPR Certification Required
- Strong communication and teamwork skills
- Compassionate, dependable, and resident-focused

Responsibilities
- Administering medications and treatments safely and accurately
- Coordinating resident admissions, discharges, and transfers
- Communicating with physicians, pharmacies, residents, and families
- Monitoring residents' conditions and reporting changes promptly
- Collaborating with the interdisciplinary care team to provide exceptional resident outcomes
- Implementing and evaluating individualized care plans
- Maintaining accurate and timely clinical documentation
- Following infection control, safety, and emergency procedures
- Supervising nursing assistants and support staff as appropriate
- Ensuring resident dignity, privacy, and rights are always protected
- Maintaining knowledge of nursing equipment and proper usage
- Providing compassionate nursing care in a long-term care and rehabilitation setting
