About The Position

Our Lady of Mercy Life Center (OLOM) is seeking a Full-Time Evening Licensed Practical Nurse (LPN) for its Sub-Acute Rehabilitation unit. This role involves providing skilled nursing care, administering medications and treatments, conducting resident assessments, and collaborating with an interdisciplinary team. Requirements

• Current New York State Licensed Practical Nurse (LPN) license.
• Graduate of an accredited Practical Nursing program.
• Strong clinical assessment and communication skills.
• Compassion, teamwork, and a commitment to providing exceptional resident-centered care.

Nice To Haves

• Long-term care, rehabilitation, or skilled nursing experience is preferred, but new graduates are encouraged to apply.

Responsibilities

• Provide skilled nursing care to residents in our Sub-Acute Rehabilitation unit.
• Administer medications and treatments in accordance with physician orders and nursing standards.
• Conduct resident assessments and report changes in condition to the RN Supervisor and healthcare team.
• Assist in developing, implementing, and updating individualized care plans.
• Educate residents and families about treatments, recovery goals, and ongoing care needs.
• Collaborate with interdisciplinary team members to support quality outcomes and successful rehabilitation.
