About The Position

The Licensed Practical Nurse (LPN) in wound care is responsible for providing direct patient care focused on the assessment, treatment, and management of acute and chronic wounds. This role supports the interdisciplinary team by performing wound assessments, implementing prescribed treatment plans, and monitoring healing progress to promote optimal patient outcomes. Key responsibilities include performing wound dressing changes, assisting with debridement procedures under supervision, documenting wound status and interventions accurately, and identifying signs of infection or complications. The LPN ensures adherence to infection control protocols, maintains wound care supplies, and educates patients and caregivers on proper wound care techniques and prevention strategies. The LPN collaborates closely with providers, RNs, and other healthcare team members to ensure continuity of care and supports quality improvement initiatives related to wound management. Strong clinical skills, attention to detail, and the ability to follow established protocols are essential in this role. Requirements

  • Current and active LPN license in the state
  • Knowledge of wound care practices, infection control, and clinical procedures
  • Strong organizational, documentation, and communication skills
  • Ability to work effectively in a team-oriented environment

  • Previous wound care experience preferred (or willingness to train)

Responsibilities

  • Perform and document wound assessments, including measurement and evaluation of healing progress
  • Provide wound care treatments, including dressing changes and application of prescribed therapies
  • Assist providers and RNs with advanced wound care procedures as needed
  • Monitor patients for signs of infection or complications and report changes promptly
  • Maintain accurate and timely documentation in the medical record
  • Educate patients and caregivers on wound care techniques, prevention, and follow-up care
  • Ensure compliance with infection control standards and wound care protocols
  • Maintain adequate inventory of wound care supplies
  • Collaborate with the interdisciplinary team to coordinate patient care
