About The Position
Presley Healthcare, LLC is looking for an experienced LPN to become our wellness nurse in our Acute Care Clinics in Jamestown/Clarkrange and Monterey Medical. This position focuses on preventive measures and visits for Medicare patients along with patient outreach for assistance with medications and social determinants of health. You will also be responsible for follow up of patients who have recently been hospitalized or in a long term care facility, such as a nursing home.
Requirements
- Up to date on licensure and certifications including BLS
- LPN (Required)
- BLS Certification (Required)
Nice To Haves
- Experience in phlebotomy
- IV therapy
- Quality measures
- 2+ years experience in a clinical setting
Responsibilities
