About The Position

Cahaba Medical Care Foundation is a community health center providing medical, pharmacy, dental, and behavioral health services to diverse underserved communities in Jefferson, Bibb, Perry, Chilton, and Wilcox counties. We are a Level 3 Patient-Centered Medical Home and Joint Commission-accredited organization, committed to increasing integration and coordination of behavioral health and primary care. This position is part of our street medicine initiative, providing care to homeless and underserved populations in the community. Requirements

• Licensed Practical Nurse (LPN) with active state license.
• Two-year degree as a Licensed Practical Nurse.
• BLS certification.
• Strong interpersonal, communication, and adaptability skills.

Nice To Haves

• Three to five years of experience in an outpatient or community health clinical setting preferred.
• Experience with vulnerable populations, community health, or street medicine preferred.

Responsibilities

• Conducts patient assessments in community settings, including obtaining weight, vital signs, and chief complaints.
• Administers injections and medications as ordered by the physician or nurse practitioner.
• Prepares mobile examination areas and necessary supplies for procedures, assisting physicians or nurse practitioners as needed.
• Collaborates with interdisciplinary teams, including CHWs and BHCs, to provide coordinated care for vulnerable populations.
• Provides patient education on disease management, preventive care, and community resources.
• Assists in referral coordination, including setting up outpatient imaging and specialty care appointments.
• Documents patient encounters accurately and timely in the electronic health record.
• Maintains safety protocols and ensures medical supplies are stocked and functional in mobile or temporary care settings.
• Participates in outreach efforts to engage homeless or underserved community members in care.
