About The Position

This position is for a Licensed Practical Nurse (LPN) in a Rehabilitation PRN role at the Hospital Sisters Health System Rehabilitation Hospital, a new 40-bed inpatient rehabilitation facility (IRF). Requirements

Current state licensure as Licensed Practical Nurse.
CPR/BCLS certification.
Excellent oral and written communication and interpersonal skills.

Nice To Haves

Graduation from an accredited Bachelors of Science in Nursing, Associate Degree in Nursing or Nursing Diploma program.
ACLS preferred.
Minimum six months’ Medical/Surgical experience in an acute care setting preferred.

Responsibilities

Provides planning and delivery of direct and indirect patient care through the nursing process of Assessment, Planning, Intervention, and Evaluation.
Develops nursing care plan in coordination with patient, family and interdisciplinary staff as necessary.
Communicates changes in patient’s clinical condition with Physicians, Nursing Supervisor/Manager, and co-workers as appropriate.
Participates in discharges planning process.
Deliver direct and indirect patient care using the nursing process—Assessment, Planning, Intervention, and Evaluation.
Collaborate closely with patients, families, and our interdisciplinary care team to create personalized care plans.
Monitor and communicate changes in patient conditions to physicians, supervisors, and colleagues.
Play a key role in the discharge planning process to support safe, successful transitions.
