Licensed Practical Nurse (LPN) - Rehabilitation Job Type: Full-time | Variable 12-hour shifts and 2 Weekends per month and major holidays. More about our team Crichton Center unit is very team-oriented—everyone works extremely well together—nursing staff and therapists. Our nurse-to-patient ratio is 1:6. All of our staff self-schedule permitting the staff more control over their schedule. Our staff meetings are often held over teams allowing staff the freedom to come in to attend or attend virtually. We have a clinical coordinator available everyday Monday through Friday who assists on the unit should they need assistance. Holidays are broken into two columns—they work one for the year, then they switch to the other grouping the following year. Our unit is a very positive unit, and you get to be a part of a patient's physical recovery which is very rewarding.

Requirements

  • Current state LPN license
  • Graduate of a practical nursing program from an accredited nursing school.
  • One-year acute care experience or other comparable nursing experience to be evaluated on an individual basis
  • Valid CPR card within thirty (30) days of hire.
  • CPR Certification renewal every two (2) years.

Responsibilities

  • Coordinates the Patient Care Team’s delivery of personalized, efficient care with respect for patient’s spiritual and cultural differences in accordance with the PA State Board of Nursing Licensure requirement and ANA Scope and Standards of Practice.
  • Adheres to the 10 Rights of medication administration.
  • Considers factors related to safety, effectiveness, and cost in delivery of patient care.
  • Assists with patient assessment throughout patient stay and participates in discharge planning maintaining therapeutic partnership with patient, family and caregiver.
  • Reinforces patient/family teaching plan.
  • Implements physician orders.
  • Reports patient’s status and anticipates situations that prevent/minimize adverse outcomes, promoting patient safety.
  • Follows established IPOC (Interdisciplinary Plan of Care) collaborating with the RN who updates as changes occur.
  • Maintains confidentiality on all patient, hospital and employee related information.
  • Demonstrates and completes annual competencies.
  • Demonstrates competency the use of restraint alternatives
  • Reviews and follows policies/procedures (i.e. infection control, safety, MSDS, and hazardous substances)
  • Adheres to schedule exhibiting flexibility when necessary and arriving at work on time
  • Assists in maintaining quality control programs
  • Demonstrates the ability to evaluate peer competency
  • Provides/documents adolescent, amputation, TBI, and spinal cord education utilizing the information provided on the unit
  • Collaborates with physician, physical therapy, occupational therapy, and speech therapy as part of Interdisciplinary Team.
  • Participates in hourly rounding and turn team
  • Documents patient’s daily goal as required in the electronic medical record
  • Updates the communication board in the patient’s room daily or as changes occur
  • Documents pertinent data in the medical record
  • Displays behaviors that promote patient safety
  • Documents as required in the care tool section in EPIC
